Appointments

Scheduling Your Appointment

Appointments can be made by calling us at 603-415-EARS (3277).  Please have ready your name, address, phone number, date of birth, insurance information and if there is a specific date or time that works best for you.

Paperwork

Patient forms can be downloaded and printed out for you to complete before your appointment.  For your convenience the forms can be mailed or faxed to us.

All forms should be completed before your scheduled appointment; otherwise new patients should arrive 20 minutes early to fill out forms.

In addition, we will need your current health insurance card and a photo ID. If you have a list of your medications, vitamins, and supplements please bring a copy with you.

All new patients please print & complete the forms below. Please contact us if you would like them mailed or faxed to you. Our privacy policy can be reviewed below. 

Day of Appointment

Our goal is to have you begin the examination process at the time of your scheduled appointment. Unfortunately, a wait is sometimes unavoidable due to unforeseen circumstances. In addition, being late for your appointment or arriving without the required insurance card or completed patient forms may result in a rescheduled or shorter appointment.

At every appointment we strive to answer all of your questions and concerns. Sometimes a patient will make an appointment for one problem and then have an expanded list when they arrive to the office. Please understand that in those circumstances we may not be able to accommodate all of your needs and you may need to schedule an additional appointment for the remaining issues.

Missed and Late Appointments

If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours’ notice. 

We understand that unforeseen circumstances may occur as you travel to your appointment. Please bear in mind that you may have a "double appointment" scheduled:  an appointment for a hearing test with the Audiologist followed by an appointment with the Ear, Nose, and Throat doctor.  If you arrive more than 15 minutes after your scheduled appointment we may, unfortunately, need to reschedule.
